Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high-quality care to their patients in a value-based alternative payment model. The driving force for ACOs is to ensure that patients, especially the chronically ill, get the right care at the right time while avoiding unnecessary duplication of services and preventing medical errors. When an ACO succeeds in both delivering high-quality care and spending healthcare dollars more wisely, it shares in the savings it achieves for the Medicare program. 👟 Join our Heart and Soles Club, a yearlong initiative dedicated to enhancing heart health awareness and bringing our senior community together! Our kick-off event will be held at Meadow Wood Mall on Saturday, May 18th from 10 am - 12 pm. Attendees can learn about volunteer opportunities, discover the benefits of walking for cardiovascular health, and join us for a walk around the mall! Guests will also be entered into our raffle for a chance to win mall gift cards. The event is open to all members of the community, so bring a friend!
